Cardiff and Vale University Health Board Patient Safety Risk after serious failings uncovered. An independent assessment into Cardiff and Vale health board raised concerns about leadership, culture, governance, planning and organisational capacity. The report was commissioned after the health board was put at the second highest level of oversight and escalation by NHS Wales in June 2025 – one below special measures.
The board recorded 16 completely preventable serious safety incidents (Never Events) from January 2024 through early 2025, with little evidence of organisational learning.
Poor building conditions and lack of digital or computer system updates led to tracking failures, such as missed diagnostic tests and lost patient follow-ups.
Staff reported feeling unable to speak openly about safety risks due to poor psychological safety, low morale, and defensive management behaviours.
