Does Asherman’s syndrome cause miscarriage?

We are often asked “Does Asherman’s Syndrome cause miscarriage”? Sadly it can. You can become pregnant if you have Asherman’s syndrome. Asherman’s syndrome causes there to be less space inside your uterus. It impacts on the embryo’s ability to embed into the wall of the uterus.

This can cause problems during pregnancy because your uterus needs to accommodate a growing baby. The placenta needs to firmly attach to the wall of the uterus. Your uterus expands and stretches throughout pregnancy. The attachment of the placenta to the wall of your uterus is very important for the growth of your developing baby.

Asherman syndrome (intrauterine adhesions or intrauterine synechiae) occurs when scar tissue forms inside the uterus and/or the cervix. These adhesions occur after surgery of the uterus or after a dilatation and curettage. Hence recurrent miscarriage treatment using D&C makes the scarring worse.

Asherman’s syndrome creates adhesions (scar tissue) that increases your risk of issues like a miscarriage, stillbirth or placenta previa during pregnancy. Placenta previa is a condition where the placenta is attached to the wall of your uterus in a very low position. It can block your cervix, blocking your baby’s way out of your uterus. This condition is related to heavy bleeding and c-section delivery.

It may occur in up to 13% of women undergoing a termination of pregnancy during the first trimester, and 30% in women undergoing a dilation and curettage (D and C) after a late spontaneous abortion. Women with placental abnormalities (e.g., placenta increta) may have a higher risk of developing Asherman syndrome as the placenta adheres to deeper layers within the uterus and becomes more difficult to remove.

The incidence may be as high as 23.4% in patients undergoing procedures two to four weeks after the initial procedure for a vaginal delivery or missed abortion. The risk increases for patients undergoing repeated procedures for bleeding or repeated elective termination of pregnancies.
